SECTION I
Rules of Eligibility for Participation
The following rules of eligibility must be observed in order to participate in the TSD interscholastic
athletic program:
A. Enrollment*
The student must be enrolled in the school by the Monday of the fourth week of the semester in which
he/she competes. The student must reside in the school boundaries in which he/she attends school and
must be enrolled in the school for which he/she competes. Student-athletes attending TCCHS and IA
East must participate in athletics at their home school based on their residency.
B. Age*
High School: A student in grades nine through twelve who participates in any interscholastic athletic
contest must be under nineteen (19) years of age. When a student’s nineteenth birthday occurs on or
after September 1 of a current school year, he/she is eligible for participation for the balance of the
school year.
Middle School: A seventh or eighth grade student must be under fourteen (14) and fifteen (15) years of
age respectively. A student who reaches that age after September 1 is eligible for participation for the
balance of that school year.
C. Create and Maintain a Parent/Guardian and Student FinalForms Account
All parents/guardians and student-athletes must complete the athletic registration process through
FinalForms, the Troy School District’s designated digital platform for athletic clearance. This includes:

A fully completed parent account and student account
Complete all electronic forms and agreements
A current physical form uploaded to the system (dated after April 15 of the previous school year)
A fully completed health history and medical profile
These items must be completed and approved in FinalForms prior to participation in any tryouts,
practices, or competitions.
It is the responsibility of the student-athlete and their parent/guardian to read and acknowledge:

The Application to Participate form
The Pay to Participate Agreement- PAID BEFORE THE FIRST COMPETITION
The Athletics Transportation Agreement form

The MHSAA Acknowledgement of Risk form
All forms and signatures will be securely stored in FinalForms and accessible to coaches, athletic
trainers, the building athletic coordinator, and district athletic director. Paper submissions are no longer
accepted unless otherwise directed by the athletic department.
